Nurse Care Manager (RN/LPN) - REMOTE MICHIGAN

Position Overview

The Remote Chronic Care Coordinator will perform telephonic encounters with patients, develop detailed care plans, and work remotely from Michigan (non-compact state). This position begins as an independent contractor with potential to transition to full-time employment with benefits.

Responsibilities
  • Conduct telephonic patient encounters and develop care plans within electronic health records.
  • Manage a patient panel of at least 100 patients within three months.
  • Meet a 90% encounter completion rate each month.
  • Participate in scheduled team meetings and adhere to the care plan and orders.
Qualifications
  • Graduate from an accredited nursing program (LPN, LVN, RN, BSN).
  • Active nursing license in Michigan (non-compact state) with no disciplinary actions.
  • At least two years of clinical experience in Med/Surg, Case Management, or home health.
  • Experience with Electronic Medical Records and Microsoft Office Suite.
  • High-speed internet and a Windows or Mac desktop/laptop (no Chromebooks or iPads).
Preferred Skills
  • Ability to recognize condition warning signs promptly.
  • Strong documentation and organizational skills.
  • Excellent communication skills.
  • Troubleshooting minor technical issues.
Compensation

Productivity-based pay structure with incentives for bilingual nurses and higher encounter volumes. Part-time hours (20-30/week) with flexible scheduling.
