Job summary

Ciena Healthcare is seeking an experienced MDS Nurse in Michigan to lead and ensure accurate Resident Assessment Instrument completion, including MDS, CAAs, and care planning in line with federal/state rules.

The role emphasizes ensuring assessments reflect residents' clinical status and supports individualized care planning, with strong emphasis on regulatory compliance, PDPM knowledge, and leadership in a team setting. RN with at least one year of assessment experience preferred.

Qualifications

• Prior MDS/RAI experience strongly preferred.
• Knowledge of RAI guidelines, PDPM, ICD-10 coding, Quality Measures, and Care Area Assessments and Care Planning processes.
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the PPS and Case Mix systems.
• Knowledge of regulatory standards and compliance requirements.
• Demonstrated ability in independent functioning and strong leadership.

Responsibilities

• Assist with accurate and timely completion of the MDS and related documentation.
• Support care planning and communications to ensure individualized resident care.
• Ensure assessments reflect residents' clinical status and comply with federal/state regulations.
