Qualifications

Responsibilities

• Complete MDS assessments within required timelines.
• Ensure accuracy of resident documentation to support care planning.
• Maintain compliance with CMS and state guidelines.
• Collaborate with interdisciplinary team for individualized care plans.
• Monitor resident progress and update care plans as needed.
• Participate in care conferences and quality improvement initiatives.
• Support reimbursement processes through accurate documentation.
• Assist with audits and surveys with accurate reports.
• Provide education on documentation standards to staff.
• Communicate with residents and families regarding care planning.
