Job summary

PACS is seeking an MDS Nurse (LVN/LPN) to coordinate and complete MDS assessments in accordance with CMS requirements. You will document resident care accurately, support reimbursement, and work with interdisciplinary teams to improve quality outcomes in our skilled nursing facility.

Ideal candidates hold a current LVN/LPN license in their state, with long-term care experience and strong knowledge of MDS 3.0, RAI processes, and EHR systems; familiarity with Medicare/Medicaid guidelines is a plus.

Qualifications

• Current LVN/LPN license in the state of practice.
• Experience in long-term care or skilled nursing facility preferred.
• Knowledge of MDS 3.0 and RAI process.
• Proficient in electronic health records (EHR) systems.

Responsibilities

• Complete MDS assessments and CAAs within required timelines.
• Ensure accuracy of resident clinical documentation for care planning.
• Maintain CMS/state/facility compliance with guidelines.
• Collaborate with interdisciplinary teams to develop care plans.
• Monitor resident progress and update care plans as needed.
• Assist with audits and surveys by providing documentation.
