Cedar River Healthcare Center in Renton, WA, is seeking a full-time MDS Nurse (LPN/LVN) to ensure residents receive accurate care plans and timely documentation. You will work with an interdisciplinary team to support quality outcomes and compliant MDS coding.

Responsibilities include completing MDS assessments per RAI and CMS guidelines, leading care plan meetings, reviewing documentation for accuracy, monitoring schedules, and collaborating with nursing, therapy, social services, and dietary

Formación

• Active LPN license in WA.
• MDS experience in skilled nursing preferred.
• Knowledge of PDPM, RAI and CMS regulations.
• Strong organization and attention to detail.
• Excellent communication and teamwork.

Responsabilidades

• Complete accurate, timely MDS assessments per RAI and CMS guidelines.
• Coordinate and lead care plan meetings with the interdisciplinary team.
• Review clinical documentation to ensure MDS coding accuracy.
