The ideal candidate is an organized LPN with experience in MDS processes and reimbursement considerations, contributing to person-centered care and continuous quality improvement.

Qualifications

• Knowledge of CMS guidelines for MDS.
• Ability to lead nursing staff and coordinate with multidisciplinary teams.
• Knowledge of long-term care nursing regulations and PPS/RUG concepts.
• CPR certification required or obtained promptly on hire.

Responsibilities

• Manage MDS assessments based on CMS guidelines.
• Coordinate, record, update, and monitor all MDS components.
• Participate in clinical meetings.
• Coordinate patient/family care conferences for residents.
• Monitor quality metrics and support QA action planning.
• Ensure timely development of MDS and care plans.
