Englewood-Health-and-Reha is seeking an MDS Nurse (LPN) to lead MDS processes in our Ohio long-term care facility. This full-time role focuses on coordinating care plans, conducting MDS assessments, and collaborating with the inter-disciplinary team.

The position emphasizes timely planning, accurate documentation, and leadership during staffing changes; CPR certification and LTC experience are valued.

Qualifications

• Requires current valid nursing license in the state of practice.
• Two years of MDS experience in long-term care.
• Demonstrates leadership and organizational skills with a positive attitude.
• Thorough knowledge of procedures and equipment in the assigned unit.
• Comprehensive nursing principles knowledge is required.
• CPR certification is required.

Responsibilities

• Acts as chairperson of the inter-disciplinary care plan team.
• Develops and updates plan of care for each resident.
• Establishes initial plan of care within five days of admission.
• Updates plan of care biweekly at minimum.
• Schedules participation of residents or guardians in care plan meetings.
• Meets daily with Therapy Director to review therapy participation and schedules.
• Notifies IDT of significant changes needing full resident assessment.
• Coordinates and leads MDS assessments and transmission dates.
• Develops an MDS assessment calendar.
• Performs MDS nursing assessments or ensures staff do so within required timeframes.
• In absence of IDT member, MDS Coordinator completes additional sections.
• Monitors IDT completion of MDS and care planning; reports issues to Administrator.
• Runs weekly Utilization Review/Medicare meetings.
• Completes monthly skilled billing log by the 8th of each month and submits to billing.
• Ensures emergency and resident care measures are used to stabilize residents.
• In staffing emergencies, performs clinical tasks as needed.
• Conducts daily stand up meetings with IDT to monitor MDS progress.
