You will lead the MDS process, collaborate with the interdisciplinary team, monitor data for trends, and ensure timely submission of documentation.

Qualifications

• Active RN license in good standing.
• Proven experience in MDS coordination and leadership.
• Strong knowledge of MDS protocols and CMS guidelines.
• Excellent organizational and communication skills.

• Proficiency with EHR and MDS software.
• Leadership experience in training staff.

Responsibilities

• Lead and coordinate the MDS assessment process to ensure accuracy and compliance.
• Collaborate with nursing, therapists, social workers, and physicians to gather resident data and facilitate care planning.
• Monitor MDS data and reports to identify trends and improvements in patient care.
• Ensure timely submission of MDS documentation to meet state and federal requirements.
• Provide education and training on MDS processes and documentation standards.
