Job summary

Thrive Rehabilitation of Pearland is seeking an experienced MDS Nurse to join our team in Pearland, Texas. As an MDS Nurse, you will conduct patient assessments, develop individualized care plans, and coordinate with the interdisciplinary team to ensure high-quality outcomes.

The role requires active Texas LVN/RN license, at least one year of MDS and long-term care experience, strong communication and customer service skills, and the ability to commute to Pearland 77584.

Qualifications

• Active TEXAS LVN/RN LICENSE (Required!)
• 1 year of MDS experience (Required!)
• 1 year of Long Term Care experience (Required!)
• Excellent customer service skills and a friendly demeanor
• Ability to commute to our facility in Pearland, TX 77584 (Required)

Responsibilities

• Conducts patient assessments utilizing the MDS instrument to identify patient needs and develop individualized care plans
• Coordinates with the interdisciplinary team to implement care plans and ensure seamless transitions
• Maintains accurate and up-to-date medical records and documentation
• Monitors patient progress and makes adjustments to care plans as needed
• Collaborates with the care team to resolve any issues related to patient care
• Participates in quality improvement initiatives to ensure optimal patient outcomes
