Zusammenfassung

Palatka Center for Rehabilitation and Healing seeks a dedicated MDS Nurse Coordinator to manage the MDS process and lead the interdisciplinary plan of care. You will coordinate the Resident Assessment Instrument and support CMS guidelines across the care team.

Responsibilities include scheduling and ensuring accurate MDS submissions, ARD setting, and maintaining compliance with OBRA, Medicare PPS, and Medicaid regulations. A strong teamwork approach is essential.

Qualifikationen

• MDS Coordinator under direction of Administrator/Director of Nursing.
• Lead daily PPS/Medicaid Case Mix meetings.

Aufgaben

• Maintain current MDS status for each patient/resident.
• Schedule OBRA/Medicare/Medicaid assessments.
• Communicate schedules to departments and families.
• Monitor patient/resident condition changes.
• Coordinate re-assessment and plan of care.
• Set ARD with interdisciplinary team.
• Complete MDS/CAAs per policies.
• Ensure compliance with federal/state regs and CMS.
