Harbor House Rehabilitation and Nursing Center in Hingham, MA is seeking a seasoned full-time, in-person MDS Coordinator to join our compassionate team. You will ensure accurate RAI completion for Medicare residents and oversee the nursing section of MDSs and RAPs in a supportive, patient-centered environment.

The role requires an active RN or LPN license, current CPR certification, and typically at least two years of MDS experience, with a strong focus on OBRA and PPS processes.

Qualifications

• Licensed RN or LPN in good standing.
• Current CPR certification.
• Minimum two years of MDS experience preferred.

Responsibilities

• Accurately complete RAI for Medicare patients in the facility.
• Complete the nursing section of all Medicare MDSs and RAPs.
• Complete OBRA MDSs as PPS caseload allows.
