Job summary

Palatka Center for Rehabilitation and Healing is seeking an MDS Nurse Coordinator to manage the RAI process, complete accurate MDS data, and lead Medicare PPS/Medicaid case-mix meetings. You will collaborate with the healthcare team to ensure CMS guidelines are followed and patient care plans are proactively updated.

The role requires coordinating schedules, maintaining documentation, and ensuring timely electronic submissions while upholding quality and regulatory standards.

Qualifications

• AHCA Clearinghouse background screening is required.

Responsibilities

• Maintains current MDS status for each patient in accordance with state and federal guidelines.
• Schedules OBRA, Medicare PPS and Medicaid required assessments.
• Coordinates and develops the plan of care as required.
• Monitors MDS and care plan documentation for complete, orderly records.
• Leads daily PPS/Medicaid case-mix meetings and ensures timely CMS-compliant submissions.

MDS Nurse Summary Description: The MDS Coordinator under the direction of the Administrator or Director of Nursing, is responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and development of the Interdisciplinary Plan of Care. Additionally, this individual will lead the daily Medicare Prospective Payment System (PPS)/Medicaid Case mix Meeting. These duties are to be performed in a timely manner consistent with Centers for Medicare & Medicaid (CMS) guidelines and policies and procedures. This individual also establishes and maintains positive relationships with all members of the healthcare team.

MDS Nurse Functions and Responsibilities:

1. Maintains current MDS status of each patient/resident, in accordance with all state and federal guidelines, including the Omnibus Budget Reconciliation Act (OBRA), Medicare PPS and any Medicaid payment system, as applicable.
2. Schedules patients/residents for appropriate OBRA, Medicare PPS and/or Medicaid required assessments.
3. Communicates schedule to all other departments and families, as required.
4. Monitors clinical systems for changes in patient/resident condition.
5. Responds to patient/resident change in condition by coordinating patient/resident re-assessment and re-evaluation plan of care.
6. Strategically sets assessment reference date (ARD), in conjunction with the interdisciplinary team, to capture needs, care and services delivered to most accurately reflect the status of the patient/resident.
7. Accurately completes portions of MDS and CAAs as assigned, in accordance. with policies, procedures, and guidelines.
8. Assures compliance with federal, state, Medicare and Medicaid regulations, and facility policies and procedures as related to completion of an accurate electronic submission in a timely manner and coordination of the RAI process.
9. Coordinates and develops plan of care as required.
10. Monitors MDS and care plan documentation on all patients/residents, assuring that complete, orderly and chronological documentation is maintained in the file.
11. Maintains Medicare Certifications /Re-Certifications
12. Participates in QAPI /QM process
13. Maintains proficiency in software programs for MDS completion and is able to access and utilize appropriate reports.
