Palm Gardens Center for Nursing & Rehabilitation is looking for an experienced MDS Coordinator (RN) who is knowledgeable and organized. Your role will involve coordinating MDS assessments and managing the assessment schedule while ensuring compliance with Medicare and Medicaid regulations.

Qualifications

• Active RN license is required.
• MDS experience in a skilled nursing facility is a must.
• Strong knowledge of MDS 3.0, RAI guidelines, PDPM, and Medicare is essential.

Responsibilities

• Coordinate and complete MDS assessments accurately and timely.
• Manage the MDS/RAI process and assessment schedule.
• Ensure compliance with CMS, Medicare, Medicaid, and regulatory requirements.
• Review clinical documentation for accuracy and completeness.
• Monitor PDPM documentation and reimbursement.
• Identify documentation opportunities and ensure appropriate follow-up.
• Participate in care planning and interdisciplinary meetings.
