Hancock Park Rehabilitation and Nursing Center is seeking a full-time in-person MDS Coordinator in Quincy, MA. The role involves accurate RAI completion for Medicare patients and completing MDSs and RAPs in a team-friendly environment.

The ideal candidate will have an active LPN or RN license, CPR certification, and at least two years of MDS experience. Join our compassionate staff in a beautiful facility near public transportation.

Qualifications

• Must have MDS experience in a care facility.
• Currently licensed LPN or RN in good standing with CPR certification.
• Two or more years of MDS experience preferred.

Responsibilities

• Complete RAI for all Medicare patients in the facility.
• Finish the nursing section of all Medicare MDSs and RAPs.
• Complete OBRA MDSs as PPS caseload allows.
