Job summary

Winchester Rehabilitation and Nursing Center in Winchester, MA is seeking a skilled MDS Coordinator to join our 120-bed facility. You will coordinate resident assessments, implement CAAs, and work with the interdisciplinary team to develop individualized care plans and ensure survey readiness.

Qualifications include a current Massachusetts RN or LPN license, at least two years of MDS and PPS experience, and strong organizational and communication skills.

Responsibilities

• Conduct and coordinate the development and completion of resident assessments.
• Conduct resident assessments in accordance with current rules, regulations, and guidelines.
• Implement resident Care Area Assessments (CAAs).
• Collaborate with the interdisciplinary care team to develop comprehensive assessments and individualized care plans.
• Participate in facility survey readiness activities and support the facility throughout the survey process.
• Ensure assessment and care planning processes are completed accurately and in accordance with applicable requirements.
• Collaborate with nursing staff and other healthcare professionals to support quality resident outcomes.
