Job Description

$5000 sign-on bonus paid within the first year for full-time!

Location: Spring Lake, MI

Under the direction of the Director of Nursing, in cooperation and collaboration with the Nursing Team, the Clinical Care Coordinator supervises the day-to-day nursing activities of the community. This supervision must adhere to current federal, state, and local standards, guidelines, regulations, and our established policies and procedures, ensuring the highest quality of care at all times.

Essential Functions
  1. Establish and maintain departmental procedures supporting operational functions.
  2. Assist in managing the department budget and ordering supplies.
  3. Determine staffing needs, participate in recruitment, and select personnel.
  4. Monitor work hours, personnel, and work assignments to ensure proper staffing and compliance.
  5. Evaluate, mentor, coach, and discipline staff; support community personnel.
  6. Participate in community surveys and develop corrective plans if needed.
  7. Oversee resident admissions, transfers, and discharges.
  8. Develop assessments of residents' nursing needs and review treatments.
  9. Ensure staff compliance with job descriptions and policies.
  10. Attend community meetings and prepare reports on nursing activities.
  11. Assist in developing and conducting staff training and continuing education.
  12. Monitor documentation practices and respond to emergencies appropriately.
  13. Maintain a clean, sanitary, and safe work environment.
  14. Participate in quality assurance and process improvement initiatives.
  15. Ensure adequate stock of medications and supplies.
  16. Ensure proper use of equipment and develop safety procedures.
Minimum Qualifications
  • Nursing degree from an accredited institution; BSN preferred (LPNs enrolled in RN program considered).
  • Active, unencumbered RN license in the state.
  • Current CPR certification.
  • Training/experience in rehabilitative and restorative nursing.
  • Knowledge of nursing practices, laws, and regulations related to long-term care.
  • Previous experience in long-term care or with geriatric populations preferred.
  • At least 1 year supervisory experience in healthcare settings.
