Job summary

RiseMe seeks a licensed nurse (LPN/LVN or RN) in Mansfield, TX to oversee the RAI and care management process from admission to discharge, following company policy and Federal, State and Certification guidelines.

Texas license required; three years’ MDS experience in a nursing facility is preferred. Training in rehabilitative nursing and knowledge of managed care rules are expected to support clinical programs.

Qualifications

• Current, active Texas nursing license (LPN/LVN or RN).
• Three years’ MDS experience in a nursing facility preferred.
• Training in rehabilitative nursing and awareness of managed care rules.

Responsibilities

• Oversee timely and accurate completion of the RAI and care management from admission to discharge per policy and federal/state guidelines.
• Coordinate information systems education for the clinical department under VP of Clinical Reimbursement.

Responsibilities

• Responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge and admission tracking, etc. With direction from the VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department.

Qualifications

• Must possess a current, unencumbered, active license to practice as an LPN/LVN or RN in this state.
• Three (3) years’ MDS experience in a nursing care facility preferred.
• Must have training in rehabilitative and restorative nursing practices.
• Must have a working knowledge of managed care rules, regulations, and guidelines.
• Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
