Job summary

Aspire Senior Living New Florence is seeking an experienced MDS Coordinator (LPN/RN) to ensure accurate assessments and individualized care plans for our residents in a compassionate setting.

You will collaborate with the care team to complete MDS reviews, ensure timely documentation, and stay up-to-date with Missouri regulations, while educating residents, families, and staff on the MDS process.

Qualifications

• Active LPN or RN license in Missouri.

Responsibilities

• Coordinate and manage MDS assessments and care plans for residents.
• Complete and submit MDS, Care Plans and required documentation on time.
• Collaborate with interdisciplinary team to meet residents' needs.
• Provide education and support to residents, families and staff on MDS and care planning.
• Maintain compliance with regulatory requirements and best practices.

MDS Coordinator (LPN/RN) - Aspire Senior Living New Florence

Aspire Senior Living New Florence is seeking a dedicated and compassionate MDS Coordinator to join our team. As a valued member of our care team, you will play a vital role in ensuring the highest quality care for our residents.

MDS Coordinator Responsibilities:

• Coordinate and manage the assessment and care planning process for residents, including the development and implementation of Individualized Care Plans
• Complete and submit accurate and timely MDS assessments, Care Plans, and other required documentation
• Collaborate with interdisciplinary team members to identify and meet the unique needs of each resident
• Provide education and support to residents, families, and staff on the MDS process and care planning
• Stay up-to-date with regulatory requirements and industry best practices to ensure compliance

MDS Coordinator Requirements:

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) is the state of Missouri
• Strong understanding of long-term care services and regulations
• Excellent communication, organizational, and problem-solving skills
• Ability to work effectively in a fast-paced environment and prioritize multiple tasks
• Strong analytical and critical thinking skills
• Ability to maintain confidentiality and discretion when handling sensitive information
