The ideal candidate is an experienced RN with at least 6 months as an MDS Coordinator, RAC-CT or RNAC certification preferred, and a compassionate, detail-oriented approach to resident care.

Qualifications

• Active RN license in good standing.
• Minimum 6+ months of experience as an MDS Coordinator.
• Experience in Skilled Nursing/Rehabilitation preferred.
• RAC-CT or RNAC certification preferred.

Responsibilities

• Coordinate and participate in MDS, CAA's, and Care Plan completion per regulations.
• Monitor Medicare/Managed Care residents with the interdisciplinary team.
• Ensure accuracy in resident assessments to maximize reimbursement.
• Drive process improvements to enhance resident outcomes.
• Maintain compliance with evolving regulations and best practices.
