Job summary

Rockdale Healthcare Center is seeking an MDS Coordinator to manage the Resident Assessment Instrument (RAI) process and ensure timely MDS completion. The role involves coordinating the interdisciplinary care plan and leading PPS/Medicaid case-mix meetings to optimize resident care.

You will maintain MDS status, schedule assessments, and coordinate with departments and families, while ensuring compliance with federal and state regulations and facility policies.

Qualifications

• Current, active nursing license in Georgia.
• RN license preferred.
• 2+ years in long-term care.
• 6 months rehabilitative/restorative care preferred.
• 2 years RAI process experience preferred.
• Ability to make independent decisions.
• Leadership and organizational skills.
• PC skills (Outlook, Office).
• Work well under pressure and meet deadlines.

Responsibilities

• Maintain MDS status for each resident in accordance with laws and facility policies.
• Schedule assessments and communicate schedules to departments and families.
• Monitor resident condition changes affecting MDS and coordinate re-assessments.
• Set ARD with interdisciplinary team to reflect resident needs.
• Complete MDS and CAAs per policies and guidelines.
• Attend daily Casemix, weekly Utilization Review, and monthly Triple Check meetings.
• Transmit MDS data electronically to state per regulations.
