Qualifications

• Current and active California LVN license.
• Experience with MDS assessments in a skilled nursing setting.
• Knowledge of CMS regulations and RAI guidelines.
• Strong clinical documentation, organizational and time-management skills.
• Excellent communication and collaboration abilities.

Responsibilities

• Complete and coordinate MDS assessments in accordance with federal and state regulations.
• Ensure MDS assessments are completed accurately and within required timeframes.
• Coordinate with the interdisciplinary team to develop and update resident care plans.
• Review clinical documentation to ensure accuracy, completeness, and compliance.
• Monitor assessment schedules, including admission, quarterly, annual, and significant change assessments.
• Support accurate RAI documentation and care planning processes.
• Collaborate with nursing, rehabilitation, dietary, social services, and other departments.
• Assist with ensuring accurate documentation and appropriate reimbursement through compliant assessment practices.
• Maintain compliance with CMS requirements, state regulations, and facility policies.
• Participate in audits, quality assurance activities, and performance improvement initiatives.
