Job summary

Studebaker Healthcare Center in Norwalk, California is seeking an experienced MDS Coordinator (LVN) to lead resident assessment data collection and ensure compliance with federal, state, and local regulations. You will coordinate with the Interdisciplinary Team to develop robust care plans and oversee timely MDS completion.

The role requires current licensure as an RN or LVN, a nursing degree from an accredited college, and strong leadership, teaching, and communication skills.

Qualifications

• Current state licensure as RN or LVN.
• Completed nursing program with a degree from an accredited college or university.
• Preferably at least 1 year in resident assessment.
• Progressive nursing experience with leadership and teaching abilities.
• Strong planning, organizing and directing skills.
• Excellent oral and written communication.
• Knowledge of elderly and/or chronically ill care needs.
• Ability to teach adult learners and use basic computer tools.

Responsibilities

• Conduct and collect information for MDS development and completion per rules and guidelines.
• Assist in implementing care area triggers and resident care plans.
• Participate in facility surveys by government agencies.
• Collaborate with IDT to develop resident assessments and care plans.
• Evaluate resident data to determine need for additional assessments or plan changes.
• Coordinate participation of outside care plan team members and families.
• Assist in scheduling timely in-service training on assessment skills.
• Ensure accuracy and completeness of assessments and understand related penalties.
• Help coordinate written preliminary and comprehensive care plans with measurable objectives.
