Studebaker Healthcare Center in Norwalk, California is seeking an MDS Coordinator (LVN) to lead the resident assessment process in line with federal, state, and local regulations. You will collect data to support the development and completion of the MDS within the Nursing Department.

The role requires strong leadership, teaching abilities, and excellent communication to work with the interdisciplinary team, schedule assessments, and ensure accurate, timely care planning.

Qualifications

• Current state licensure (RN or LVN) required.
• Completed nursing program and degree from an accredited college/university.
• Preferably 1 year in resident assessment position.
• Strong leadership, teaching and communication skills.
• Knowledge of elderly/chronically ill care; infection control basics.

Responsabilités

• Conduct and collect data to develop and complete the MDS per rules and guidelines.
• Assist in implementing care area triggers and resident care plans.
• Participate in facility surveys by government agencies.
• Collaborate with IDT to develop resident assessments and care plans.
• Evaluate residents' data to identify need for additional assessments or changes to admissions.
