Smith Ranch Skilled Nursing and Rehabilitation in San Rafael, CA seeks an LVN MDS Coordinator to input MDS information and support the facility’s MDS processes. The role includes on-floor duties a couple of days weekly, with emphasis on accuracy and timelines.

The candidate must hold a CA LVN license, have MDS Coordinator experience in LTC, be MDS certified, and be proficient with MDS 3.0 reimbursements. Strong typing (50 wpm) is required.

Qualifications

• CA LVN license required.
• Minimum 1 year as MDS Coordinator in LTC setting.
• MDS Certified.
• Knowledge of PDPM.
• Proficient in MDS 3.0 reimbursements.
• Typing speed of at least 50 words per minute.

Responsabilités

• Enter MDS data into the MDI system by deadlines with high accuracy.
• Compare data against original MDS documents and correct as needed.
• Retrieve and print initial and final validation reports; lock MDS after validation.
• File MDS documents in residents' charts.
• Notify coworkers of questions or delays related to MDS data.
• Maintain confidentiality of patient information.
