Job summary

Complete Care is seeking an experienced MDS Director (RN) to oversee the MDS process for a New Jersey skilled nursing facility. The role requires expert knowledge of MDS, PDPM, and reimbursement, with leadership responsibility for the MDS team.

You will coordinate assessments, ensure CMS compliance, manage care planning and documentation, and partner with nursing leadership to improve resident outcomes. Strong organizational and communication skills are essential.

Qualifications

• Active New Jersey RN license or valid Compact RN license required.
• Prior MDS experience in a skilled nursing facility required.
• RAC-CT (Resident Assessment Coordinator - Certified) certification strongly preferred.
• Strong knowledge of MDS 3.0, PDPM, Medicare, and Medicaid reimbursement.
• Previous MDS leadership or supervisory experience preferred.
• Strong organizational, communication, and leadership skills.
• Experience with PointClickCare preferred.

Responsibilities

• Oversee and coordinate the MDS assessment process, ensuring accuracy and timely completion.
• Ensure compliance with CMS guidelines and state and federal regulations.
• Manage care planning, documentation, and reimbursement accuracy.
• Collaborate with nursing leadership and interdisciplinary teams to support quality resident care.
• Monitor quality measures and identify opportunities for improvement.
• Provide guidance and education to staff regarding MDS processes and requirements.
