Job summary

SNF Staffing Solutions seeks a skilled MDS Coordinator to manage the MDS process from admission to discharge, ensuring alignment with Federal, State, and Certification guidelines. You will coordinate with the DON and VP of Clinical Reimbursement to support accurate coding and care planning.

The role emphasizes interdisciplinary collaboration, timely data entry, and adherence to regulatory requirements within a nursing facility setting.

Qualifications

• Graduate of an approved Licensed Vocational program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Excellent knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process, OBRA regulations and MDS.
• Knowledge of the care planning process.
• Experience with MDS 3.0 is preferred.

Responsibilities

• Coordinate the MDS assessments and care plan reviews with the Interdisciplinary Team.
• Ensure the Interdisciplinary team decisions for MDS and assessments meet clinical criteria.
• Maintain MDS status and data entry in the medical record.
• Complete and submit required documentation to state databases per policy.
• Participate in regulatory surveys and quality assurance activities.
