Job summary

OPCO Skilled Management’s Forest Park Nursing Center seeks an experienced MDS Coordinator to manage the RAI and care management process from admission to discharge in alignment with policy, Federal, State, and Certification guidelines.

Qualified candidates will be RN or LVN licensed in the state of practice, with at least 2 years in a Skilled Nursing Facility and strong knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA, and MDS 3.0.

Qualifications

• RN/LVN license in the state of practice.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, Medicare PPS and Medicaid reimbursement.
• Understanding OBRA and the Minimum Data Set (MDS) process.
• Experience with MDS 3.0 is preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments.
• Decide on whether to complete additional MDS assessments based on criteria.
• Coordinate daily stand-up meetings and set assessment reference dates.
• Ensure compliance with federal and state RAI regulations.
• Monitor MDS and care plan documentation in the medical record.
• Maintain current MDS status per guidelines.
• Ensure accurate data entry into MDS systems.
• Perform MDS coding using medical records, observations, and interviews.
• Other duties as assigned.
