The Columbia Presbyterian Community is seeking a dedicated MDS Coordinator to ensure accurate MDS assessments, care plans, and regulatory compliance. You will collaborate with an interdisciplinary team to deliver exceptional care to residents, while supporting quality initiatives and ongoing education.

Qualifications

• Licensed Practical Nurse or Registered Nurse in South Carolina.
• Two years of clinical experience in healthcare.
• One year of experience in long-term care.
• One year of experience as MDS Coordinator.
• Strong clinical assessment skills.
• Intermediate computer skills.
• CPR certification.
• Valid SC Driver’s License.

Responsabilités

• Lead and oversee clinical care programs in compliance with standards and federal guidelines.
• Educate staff on the survey process and ensure compliance readiness.
• Oversee individualized care plans aligning with resident wishes and directives.
• Drive quality improvement through QA initiatives.
• Assist admissions by assessing resident needs for placement.
• Coordinate interdisciplinary team for RAI implementation per rules.
• Ensure accurate assessments and care plans via interdisciplinary review.
• Support Medicare coverage decisions and ARD scheduling.
• Disseminate updates on the RAI process to the team.
• Ensure timely electronic submission and validity of MDS data.
