Job summary

River Grove Health and Rehab is seeking an MDS Coordinator (RN or LPN) to lead MDS/RAI processes and ensure compliance with federal and state guidelines. The role requires MDS certification and an active nursing license, with at least 3 years in long-term care.

Qualifications

• Active RN/LPN license with current state licensure.
• MDS 3.0 experience or certification.
• CPR certification current.

Responsibilities

• Ensure utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's.
• Observe direct nursing care, review documentation and make appropriate recommendations, assist with chart audits.
• Provide clinical support and direction to appropriate resource materials.
• Verify all MDS documents required by Federal/State Guidelines or Organizational policy are available and completed timely.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings for federal billing compliance.
• Assist in completing the Resident Assessment Instrument with the Interdisciplinary Team.
• Assist with coordination and completion of the Resident Assessment Instrument per regulations.
