CCH Healthcare is seeking an experienced MDS Coordinator (RN or LPN) in Cincinnati, OH. The role focuses on overseeing the entire MDS process, CAAs, and care plans to ensure compliance and accurate reimbursements.

You will lead interdisciplinary team meetings, monitor quality measures, and support Medicare PPS and Medicaid processes in our Cincinnati area facilities. Housing and comprehensive benefits are offered.

Qualifications

• Active, unencumbered RN or LPN license in Ohio.
• Minimum 2 years of MDS experience in a long-term care setting.
• CPR certification.
• RAC-CT certification preferred.
• Strong knowledge of Case-Mix, Medicare PPS, and regulatory compliance.

Location: Cincinnati, OH (CCH Healthcare Network)

CCH Healthcare is currently hiring experienced MDS Coordinators for our Cincinnati area facilities. We’re looking for passionate and detail-oriented nurses who excel in clinical documentation, team collaboration, and resident-centered care.

About the Role

As an MDS Coordinator, you will be responsible for overseeing the entire RAI process to ensure timely and accurate completion of MDS assessments, Care Area Assessments (CAAs), and care plans. This role is essential to compliance, clinical excellence, and reimbursement accuracy within our facility.

Key Responsibilities

• Coordinate and manage MDS assessment and submission processes
• Ensure accuracy and timely state transmission of MDS data and reports
• Lead interdisciplinary care plan meetings
