Job summary

CareCore Health is seeking an experienced MDS Coordinator to oversee the RAI process and support accurate clinical reimbursement while enhancing resident care in a skilled nursing setting.

The role requires Ohio RN or LPN licensure, familiarity with PDPM/Medicare/Medicaid, and strong assessment/communication skills. Collaboration with interdisciplinary teams and attention to regulatory compliance are key parts of the job.

Qualifications

• Current Ohio RN or LPN license required.
• MDS RAC-CT certification preferred.
• Experience with MDS in skilled nursing or long-term care setting.
• Working knowledge of Medicare, Medicaid, and PDPM.
• Strong assessment, documentation, and organizational skills.
• PCC experience is a plus.
• Excellent communication and teamwork skills.

Responsibilities

• Coordinate and complete MDS assessments in accordance with federal and state regulations.
• Ensure timely and accurate completion of the RAI process.
• Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members.
• Monitor Medicare and Medicaid documentation to support reimbursement.
• Participate in care plan meetings and interdisciplinary team conferences.
• Review clinical documentation for accuracy and compliance.
• Assist with quality improvement initiatives and regulatory readiness.
• Maintain current knowledge of CMS regulations and reimbursement guidelines.

We are seeking an organized, detail-oriented MDS Coordinator to oversee the Resident Assessment Instrument (RAI) process and help ensure accurate clinical reimbursement while supporting quality resident care.
