CareCore at Lakeridge seeks a dedicated full-time MDS Coordinator to ensure MDS compliance, manage RUGs/PDPM, and optimize Medicaid/Medicare processes for our residents.

The ideal candidate is a licensed nurse with MDS knowledge, strong communication, and proficiency in EHR/MDS software, capable of working collaboratively in a team.

Qualifications

• Licensed Nurse in Ohio (RN preferred).
• RAC-CT certification preferred.
• 1 year minimum experience as MDS Coordinator or equivalent.

Responsibilities

• Ensure compliance with federal and state guidelines for MDS.
• Manage Medicaid/Medicare RUGs/PDPM and Case Mix.
• Prepare documentation to meet Medicare criteria and support Quality Measures.

Position Overview:

The MDS Coordinator will play a crucial role in ensuring compliance with federal and state guidelines related to the Minimum Data Set (MDS), and will be responsible for managing Medicaid and Medicare RUGs/PDPM, Medicaid Case Mix, and meeting Medicare criteria, including all required supportive documentation. A strong understanding of Quality Measures and the Five-Star Quality Rating System is essential.

Qualifications:

• Licensed Nurse in the State of Ohio (RN Preferred)
• RAC-CT Certification Preferred
• Minimum of 1 year of experience as an MDS Coordinator or equivalent role
• Strong understanding of MDS 3.0 guidelines and skilled nursing facility regulations
• Knowledge of the Medicare and Medicaid reimbursement process
• Excellent communication, organizational, and time management skills
• Proficiency in electronic health records (EHR) and MDS software systems
• Ability to work collaboratively in a team-oriented environment
