Job summary

Carrier Mills Nursing & Rehabilitation Center is seeking an experienced MDS Nurse Manager (RN or LPN) to join our leadership team. This role coordinates MDS assessments, leads care planning, and ensures accurate documentation across shifts.

Enjoy a Monday–Friday schedule from 8:00 AM to 4:30 PM, with a $10,000 sign-on bonus and competitive RN/LPN wages in the $33–$35/hour range. This is a full-time role with opportunities to improve resident outcomes and facility performance.

Qualifications

• Current Illinois RN or LPN license.
• Experience with MDS and care planning.
• Long-term care experience preferred.
• Knowledge of Medicare, Medicaid, and reimbursement.
• Strong organizational and communication skills.
• Ability to collaborate with an interdisciplinary team.

Responsibilities

• Coordinate and complete MDS assessments.
• Lead and support interdisciplinary care planning.
• Monitor documentation, case mix, and reimbursement.
• Support quality measures and survey readiness.
• Maintain CMS and regulatory compliance.

Compensation

• Licensed Practical Nurse (LPN): $29.00–$31.00/hour
• $10,000 Sign-On Bonus
• Sign-on bonus paid over one year
• Annual performance evaluation with the potential for a pay increase
