Job summary

iCare Health Network in Holyoke, MA seeks an MDS Coordinator to oversee Medicare/Medicaid case-mix documentation, MDS completion, and alignment with PPS/OBRA requirements to optimize reimbursement. This salaried role coordinates care across nursing, dietary, social services and rehabilitation teams.

The position reports on a Monday–Friday schedule with starting salary around $65,000 per year, with potential for higher compensation based on RN/LPN credentials and experience in long-term care.

Qualifications

• Active, unencumbered MA RN or LPN license.
• Knowledge of QA/Quality Improvement processes.
• Experience supervising in long-term care preferred.

Responsibilities

• Coordinates the assessment, planning, implementation and evaluation of resident care by following the MDS process.
• Coordinates and facilitates care conferences with the interdisciplinary team.
• Coordinates, completes and reviews Minimum Data Sets (MDS).
• Educates, trains and assists nursing staff and other departments regarding the roles of assessments, planning, implementation and MDS.
• Responsible for ensuring all current MDS/PPS updates are implemented and staff is in-serviced.
• Cross train between PPS and OBRA.
• Conducts regular resident interviews as required by the MDS.
• Available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed.

MDS Coordinator

The primary role of the MDS Coordinator is to coordinate and guide the overall process and tracking of all Medicare/Medicaid case-mix documents, and care assessments in order to assure appropriate reimbursement for services. The position is also responsible for completing concurrent MDS to assure achievement of maximum allowable RUG categories, as well as integrating information from Nursing, Dietary, Social Services, and Rehabilitation departments for appropriate reimbursement.

Schedule & Pay:

• Starting salary: $65,000 / year
• Monday - Friday, flexible hours as it is a salaried position
