Pikes Peak Post Acute in Ohio is hiring an experienced MDS Nurse (RN/LPN) for a full-time role in a long-term care facility. The position offers a competitive salary of $80,000 per year and the chance to impact resident care through accurate MDS assessments and collaborative care planning.

You will coordinate MDS activities, ensure timely documentation for reimbursement, and support quality and regulatory compliance while working with an interdisciplinary team. Licensure as RN or LPN is required.

Qualifications

• Current RN or LPN licensure in good standing.
• Experience with MDS processes in long-term care settings preferred.
• Strong knowledge of Medicare/Medicaid and PPS.
• Excellent organizational and communication skills.
• Ability to work independently and with the interdisciplinary team.

Responsibilities

• Coordinate and complete MDS assessments per regulations.
• Ensure accurate and timely submission of MDS documentation.
• Collaborate with interdisciplinary teams to develop/update resident care plans.
