Job summary

PACS Group is seeking an MDS Nurse (LVN/LPN) to coordinate and complete MDS assessments in compliance with federal and state regulations. You will ensure accurate documentation, support reimbursement processes, and collaborate with interdisciplinary teams to promote quality outcomes for residents.

The role requires an active LVN/LPN license, knowledge of MDS 3.0/RAI, and proficiency with EHR systems and Microsoft Office.

Qualifications

• Must hold a current LVN/LPN license in the state of practice.
• Knowledge of MDS 3.0, RAI processes, and Medicare/Medicaid reimbursement guidelines.
• Proficiency with electronic health records (EHR) and Microsoft Office.
• Strong organizational skills and attention to detail; able to work with interdisciplinary teams.

Responsibilities

• Complete MDS assessments and CAAs within required timelines.
• Maintain accurate resident documentation to support care planning and reimbursement.
• Ensure CMS/state/facility regulatory compliance.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Monitor progress and update care plans as needed; participate in care conferences.
• Support audits and surveys with accurate documentation and reports.
