Job summary

Briarwood Nursing and Rehab in Flint, MI is seeking an experienced MDS Coordinator/NAC (RN or LPN) to lead the RAI process, ensure accurate and timely assessments, support compliant documentation, and help maximize resident outcomes and reimbursement.

You will collaborate with an interdisciplinary team to develop individualized care plans, stay current on regulatory changes, and contribute to a supportive, growth‑oriented workplace that values quality and teamwork.

Qualifications

• Current RN license in Michigan. Exceptional LPNs will be considered.
• At least two years of MDS 3.0/RAI experience preferred.
• RAC-CT certification preferred.
• Strong organizational skills and attention to detail.

Responsibilities

• Lead and manage the completion of MDS assessments in compliance with federal and state regulations.
• Collaborate with the interdisciplinary team to develop individualized care plans reflecting residents' needs and goals.
• Ensure documentation and coding are accurate, complete, and aligned with reimbursement requirements.
• Review clinical data to identify trends and support quality improvement.
• Stay current on regulatory updates and MDS best practices.
