Job summary

Christian Living Communities is seeking an MDS Coordinator (RN/LPN) for an on-site role in Centennial. The position is full-time with a clear pay range; final compensation depends on licensure and experience.

The role coordinates resident assessments and care plans, supports PPS/OBRA processes, and collaborates across departments while ensuring compliance with state requirements. Responsibilities include MDS transmission, care plan conferences, and participating in surveys, with nursing duties

Qualifications

• RN license in the state of operation and current licensure
• Minimum 2 years of managerial experience in hospital or nursing care
• Ability to assess age-specific needs of residents
• Flexibility with rotating on-call responsibilities
• Ability to problem solve independently with knowledge of medical practices and regulations
• Proficiency with Word, Outlook and Excel

Responsibilities

• Oversee and coordinate assessments and care plans with multiple departments
• Monitor PPS, OBRA, RAPs and Triggers completion and updates
• Contribute to policy and procedure updates for MDS and care plans
• Transmit completed MDS weekly and maintain 15 months of MDS records
• Assess prospective residents with the team and work on an interdisciplinary basis
• Perform nursing functions as needed and handle administrative duties
• Develop and maintain a quality assurance program for resident assessment and care plans
• Participate in on-site surveys by government agencies
• Execute other assigned duties
