Qualifications

• Bachelor's degree or state-certified certification required.
• LPN or RN licensure required.
• Valid CPR certification required.
• 3–5 years of MDS/assessment experience in long-term care environment.
• Proficient with Microsoft Office Suite.

Responsibilities

• Lead the MDS assessment process in accordance with regulatory requirements and guidelines.
• Interview residents for MDS assessments.
• Ensure timely completion of baseline and comprehensive care plans.
• Maintain accuracy and compliance in MDS coding and assessments.
• Prepare and transmit reports to Medicare/Medicaid databases.
