Waters Edge Health and Rehabilitation Center seeks a compassionate MDS Coordinator to join our team. You will assess resident health, collect and analyze data, and develop individualized care plans aligned with each resident’s needs and expectations.

This role blends clinical and administrative responsibilities focused on quality care and compliance. This full-time position runs Monday through Friday day hours, with duties including comprehensive assessments, care plan updates, and collaboration

Qualifications

• RN or LPN license in good standing
• MDS experience in long-term care preferred
• Strong communication, organizational, and analytical skills

Responsibilities

• Conduct comprehensive assessments of residents
• Develop and update resident care plans
• Monitor patient care through assessments, observation, and medical record reviews
• Collaborate with clinical teams to ensure accurate documentation and effective care delivery
• Create and manage medical codes for billing and recordkeeping
