Job summary

Willow Ridge Rehabilitation & Living Center is seeking an experienced RN/LPN MDS Coordinator to take ownership of the MDS/RAI process, ensuring accurate assessments and compliant documentation. You will work with an interdisciplinary team to support individualized care plans and reimbursement accuracy.

Qualifications

• Active and unrestricted RN or LPN license.
• Minimum of 2 years of MDS experience in long-term care or skilled nursing.
• Strong knowledge of the MDS and RAI process.
• Experience with care planning and clinical documentation.
• Knowledge of Medicare reimbursement and applicable long-term care regulations.
• RAC-CT certification preferred.
• Strong organizational, communication, and time-management skills.
• Ability to manage multiple deadlines and priorities.
• Team-oriented approach with a commitment to quality resident care.

Responsibilities

• Coordinate and ensure timely and accurate completion of MDS assessments.
• Oversee the RAI process and maintain compliance with applicable requirements.
• Review clinical documentation for accuracy, completion, and regulatory compliance.
• Manage MDS transmissions and validate required submissions.
• Coordinate and participate in interdisciplinary care planning.
• Monitor Quality Measures (QM) and identify opportunities for improvement.
• Collaborate with nursing, therapy, providers, and facility leadership.
• Ensure resident care plans reflect individual needs, goals, and services.
• Support accurate reimbursement through proper assessment and documentation practices.
• Maintain required MDS documentation and meet established deadlines.
