• Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing.
• Previous MDS experience in long-term care is preferred.
• Strong communication, organization, and analytical skills.

Responsibilities

• Conduct comprehensive assessments of residents.
• Develop and update resident care plans.
• Monitor patient care through assessments, observation, and medical record reviews.
• Collaborate with clinical teams to ensure accurate documentation and effective care delivery.
• Create and manage medical codes for billing and recordkeeping.
