Barbourville Health & Rehabilitation is actively seeking an experienced MDS Coordinator to join our Kentucky long-term care team. The role focuses on preparing and overseeing MDS assessments, care plans, and coordinating with physicians and nursing staff to ensure regulatory compliance.

The ideal candidate holds an RN or LPN license in Kentucky, has long-term care experience, and demonstrates leadership, organization, and meticulous attention to documentation and resident care.

Qualifications

• Current license as RN or LPN in Kentucky.
• Long-term care experience is highly desired.
• Ability to manage complex documentation and care plans.
• Strong communication with residents and health care team.

Responsibilities

• Coordinate and prepare MDS assessments for all residents.
• Oversee the transmission of MDS resident assessments.
• Prepare, monitor, and update assessments, Care Area Assessments, and Care Plans.
• Participate in Care Plan meetings with residents and family members.
• Assist with admission and discharge assessments and related paperwork.
• Assist/direct visitors and support nursing staff to meet resident needs.
