Job summary

Sprenger Health Care Systems in Avon Lake, OH seeks an LPN to coordinate RAI processes and manage OBRA/PPS assessments for residents. The role requires strong computer skills, MDS knowledge (preferred), and solid organizational and communication abilities.

Qualifications

• LPN required, computer knowledge necessary.
• Knowledge of MDS preferred.
• Good organizational and follow-through skills.
• Strong communication skills required.
• Ability to be flexible with work hours as needed.
• Experience in long term care preferred.

Responsibilities

• Coordinate the RAI process for all assigned residents according to state and federal regulation.
• Schedule all OBRA required and PPS assessments according to state and federal regulation.
• Complete data entry of information from residents' assessments into the Electronic Charting System.
• Assure accurate completion of all assessments.
• Complete and track all admissions, discharges and re-entries to the facility.
• Schedule Plan of Care Meeting for all residents.
