Job summary

Sprenger Health Care is seeking an LPN to coordinate the RAI process for residents at our Avon Lake facility. You will schedule OBRA and PPS assessments, enter data into the electronic charting system, and ensure accurate completion of all assessments while tracking admissions, discharges and re-entries.

The role requires excellent organizational and communication skills, flexibility with hours, and prior long-term care experience.

Qualifications

• LPN license required with good communication skills.
• Computer knowledge required; familiarity with MDS preferred.
• Knowledge of MDS preferred and ability to work with regulatory requirements.
• Good organizational and follow-through skills.
• Strong communication abilities with residents, families, and staff.
• Flexibility with work hours as needed; experience in long-term care preferred.

Responsibilities

• Coordinate the RAI process for all residents per state and federal regulations.
• Schedule OBRA-required and PPS assessments according to regulations.
• Complete data entry of assessment information into the Electronic Charting System.
• Ensure accurate completion of all assessments and documentation.
• Complete and track admissions, discharges, and re-entries to the facility.
• Schedule Plan of Care Meetings for all residents.

Qualifications:

• LPN, computer knowledge required, knowledge of MDS preferred, must have good organizational and follow through skills, must have good communication skills, ability to be flexible with work hours as needed, experience in long term care preferred
