Job summary

River Grove Health and Rehab is seeking an LPN or RN for our MDS position. Join a supportive team focused on quality care for residents and compliance with Federal and State regulations.

Responsibilities include coordinating MDS/RAI with the interdisciplinary team, reviewing documentation, and ensuring timely completion of required forms. A current RN/LPN license, MDS certification, and CPR are required; 3 years of healthcare experience, preferably long-term care, is preferred.

Qualifications

• Active RN/LPN license.
• Certification in MDS.
• MDS 3.0 experience.
• 3+ years of clinical experience in health care; long-term care preferred.
• Current CPR certification.

Responsibilities

• Utilize interdisciplinary rehab team process in formulation of MDS/RAIs.
• Observe direct nursing care, review documentation and make recommendations.
• Provide clinical support and direction to resource materials.
• Ensure MDS documents comply with Federal/State guidelines and are completed timely.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month-end meetings.
• Assist in completing the Resident Assessment Instrument with the Interdisciplinary Team.
• Coordinate and complete the Resident Assessment Instrument per current regulations.

Responsibilities of the MDS Coordinator:

• Ensure utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's.


• Observe direct nursing care, review documentation and make appropriate recommendations, assist with chart audits.


• Provide clinical support and direction to appropriate resource materials.


• Verify all MDS documents required either by Federal and/or State Guidelines or Organizational policy are easily available at all times is completed timely as set forth by Federal Regulations.


• Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met.


• Assist in completing the Resident Assessment Instrument with the Interdisciplinary Team.


• Assist with the coordination and participate in the completion of the Resident Assessment Instrument in accordance with current Federal and State Regulations.

Requirements of the MDS Coordinator:

• Active RN/LPN license


• Certification in MDS


• Licensed Practical/Vocational Nurse with required current state licensure.


• Minimum three (3) years of clinical experience in a health care setting; long-term care setting preferred.


• Must have a current/active CPR certification.


• Must know MDS 3.0 Experience


• Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
