Job summary

Responsibilities

• Ensure utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's.
• Observe direct nursing care, review documentation and make appropriate recommendations, assist with chart audits.
• Provide clinical support and direction to appropriate resource materials.
• Verify all MDS documents required by Federal/State Guidelines or Organizational policy are available and completed timely.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings for federal billing compliance.
• Assist in completing the Resident Assessment Instrument with the Interdisciplinary Team.
• Assist with coordination and completion of the Resident Assessment Instrument per regulations.

Responsibilities of the MDS Coordinator:

• Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met.

• Assist in completing the Resident Assessment Instrument with the Interdisciplinary Team.

• Assist with the coordination and participate in the completion of the Resident Assessment Instrument in accordance with current Federal and State Regulations.

Requirements of the MDS Coordinator:

• Active RN/LPN license

• Certification in MDS

• Licensed Practical/Vocational Nurse with required current state licensure.

• Minimum three (3) years of clinical experience in a health care setting; long-term care setting preferred.

• Must have a current/active CPR certification.

• Must know MDS 3.0 Experience

• Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
