Job summary

Alpine Care of St. Charles is seeking an experienced MDS Coordinator RN/LPN to join our team in St. Charles, IL. You will coordinate the MDS assessment process with an interdisciplinary team to ensure accurate data and regulatory compliance.

This role requires current Illinois RN or LPN licensure and at least 1 year of healthcare experience, preferably in long-term care. You will educate staff, maintain records, and support a patient-centered, quality-focused environment.

Qualifications

• Current RN or LPN licensure in Illinois.
• Minimum of 1 year of healthcare experience, preferably in long-term care or rehabilitation.
• Strong understanding of MDS assessment processes and regulations.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct MDS assessments and reviews to ensure accurate data collection and reporting.
• Coordinate with interdisciplinary team members to gather information and complete MDS assessments.
• Ensure all MDS assessments are completed timely and meet regulatory requirements.
• Develop and implement strategies to improve MDS accuracy and completeness.
• Provide education and training to staff on MDS processes and procedures.
• Maintain accurate and up-to-date records and reports.
• Collaborate with other departments to ensure seamless communication and coordination.
