Qualifications

• Illinois RN license required.
• Graduate of accredited nursing program.
• CPR certification.
• Experience as MDS Coordinator preferred.
• 1 year RN in Skilled Nursing Facility.
• 1 year CNA experience preferred.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan.
• Perform resident assessments and assist in discharge process.
• Ensure residents are comfortable and safe through attention to detail.
• Support team with MDS coordination.
