Job summary

The Waters in Indianapolis, IN is seeking an MDS Coordinator/Nurse to perform federally mandated assessments of residents in a long-term care facility. As an RN, you will collect data and compile assessments to inform care plans and determine Medicare/Medicaid reimbursements.

Key duties include OBRA/PPS/MDS completion, care plan development with interdisciplinary teams, timely scheduling of conferences, CMS transmissions, and maintenance of MDS-related documentation in the EMR.

Qualifications

• RN license required for the MDS Coordinator position.
• Optional MDS Certification from AANAC is a bonus.

Responsibilities

• Complete OBRA, PPS and Managed Care MDS.
• Complete Admitting MDS Tracking Forms, Death in Facility tracking and required Discharge Assessments per the RAI Manual.
• Coordinate Care Plan development with other disciplines and OBRA schedule.
• Schedule and conduct Care Plan Conferences within 21 days of admission and every 90 days thereafter.
• Prepare Care Plan conference letters and manage invitations to residents and families.
• Maintain monthly OBRA calendar by the 20th of each month.
• Oversee weekly OBRA, PPS and Care Plan schedules for the IDT.
• Transmit OBRA/PPS MDS to CMS per guidelines.
• Coordinate and certify Medicare Part A/Managed Care obligations.
• Manage AB Notices and Medicare Cut Letters.
• Maintain the 100-day Medicare Part A and Managed Care log.
• Oversee Weekly Medicare Part A/Managed Care and RUG-IV 48 reports.
• Organize RUG-IV 48 supporting documentation and file organization.
• Complete ICD-10 DX module within the EMR within 72 hours of admission and on re-admissions.
• Coordinate CAAs for Full Comprehensive OBRA Assessments.
• Coordinate the 4 MDS interviews (BIMS, PHQ-9, Pain, Activity) and sign off on ARD.
• Coordinate ancillary departmental assessments for validation and ARD alignment.
• Present at Weekly Medicare/Managed Care and RUG-IV meetings.
• Manage insurance/managed care/Medicare caseload and re-authorization for services.
• Complete end-of-month billing and review as part of Triple Check.
• Monitor for Significant Changes in Status via 24-hour report and trigger new MDS if needed.
• Oversee EMR monitoring of ADLs, restorative programs, mood/behaviors.
• Document in POC to ensure accurate ADL coding; print/analyze QM/QI reports.
• Participate in QI/QM meetings and audits as assigned.
• Coordinate MDS-related surveys (focus and general) as needed.
• Assist with ADR requests for Medicare Part A/B and Managed Care.
• Perform other MDS responsibilities per direction of the MDS Consultant.
